For Medicare to cover a power mobility device the patient must have a face to face evaluation with a physician. The evaluation must be for the purpose of prescribing power mobility, and it must be noted in the patients chart. The process is the same regardless of the device being ordered (power chair or scooter).
There are a set group of criteria to be addressed in the evaluation. Specifically, the Mobility Related Activities of Daily Livings (MRADL’s) should be addressed. Note how their current condition limits their ability in: feeding, bathing, grooming, dressing, toileting, or moving from room to room. These notes should be entered into the patients chart.
